Confidentiality and Limits
Everything you share with me is private and confidential. There are a few situations where I may be required to share information:
- If there is a serious risk of harm to you or someone else
- If I am required to make a mandatory report (for example, if a child is at risk)
- If disclosure is required by a court or legal authority
- To contact someone you trust if you are seriously ill or unable to make decisions
- If the College of Registered Psychotherapists of Ontario (CRPO) requests information for a complaint or review
Before starting therapy, I will explain these limits and answer any questions you have.
Storage and Security
I take your privacy seriously and use secure methods to protect your information:
- Digital records are stored on encrypted platforms (e.g., Jane app) and password protection
- Physical records are kept securely
- Access is limited to authorized personnel only
Record Retention
Your therapy records are kept for at least 10 years after your last session, or 10 years after a minor turns 18, whichever is later, in compliance with professional standards.
Your Rights
You have the right to:
- Access your records
- Request corrections
- Withdraw consent for certain uses of your information
- Ask questions about how your information is used
- File a complaint with with the Information and Privacy Commissioner of Ontario
